Eligible City of Eugene employees are generally entitled to twelve weeks of unpaid family or medical leave during any 12-month period, as provided under the federal Family and Medical Leave Act (FMLA) of 1993, and under the State of Oregon Family Leave Act (OFLA, ORS 659.010 and 659.121).
In addition, eligible employees may be entitled to paid leave under the City's Paid Family and Medical Leave (PFML) equivalent plan. More information on paid benefits is available on the City's Paid Family and Medical Leave webpage.
To begin the process for FMLA/OFLA leave, complete a Family and Medical Information Form and return to COEBenefitsLeaveTeam@ci.eugene.or.us. Once your form is processed, the Leaves staff will provide your designation that may have additional steps for you to complete.
